Help with apparel form

1.

Select your organization from the drop down list.

2.

Click on Item #1 Item Description and type in the name and/or number of your first item (for example, F1- AKA Figurine).

3.

Enter the price of the item, as shown on the display page.

4.

Enter the quantity of items ordered.

5.

Enter total price for the quantity of items ordered.

6.

For additional items, go to Item #2 and repeat the process above.

Up to three items can be ordered per form. If you want to order additional items, complete a new form page, type in your name, and add “continued” in the comment section.

Afghans can be personalized with your name, chapter, initiation date, line name, etc. for the additional price of $10 per line of writing (3 lines maximum). Just add “monogram” as the item description on your order form, and add your information in the comments section of the form.

When you have finished completing each form, indicate total amount of all items submitted on that page. We will add up the items from all of your forms, and contact you with the total charges, including shipping.
